Is poison crab always take over host?
The problem with Poison zombie is that it WILL take over your host if their health is under 100 for some reason, i tested with 105 health and it doesn't take me instantly, but it does when my health is at 100. Maybe some conflicts or bugs causes it but i have not seen any in the console.

Since poison headcrabs can't kill their targets, if you don't have the option for headcrabs to always instantly take over suitable hosts, they will instead have a random chance to take over an enemy every time they bite it. The target's health shouldn't affect this process the way you described it, so it's almost definitely some sort of mod conflict.
