The biggest risk of small-the-back (SOB) carry isn't negligent or accidental discharges (ND and AD) though you do have a increased risk of the former, but spinal injuries from falling. It's also a slow and not particularly safe draw that isn't well suited to asymmetric positions. Pretty much the only people who SOB carry these days are people who learned it from pop culture and don't know any better. Anyone who's serious about gun handling doesn't do it. That said, it was much more popular and much less frowned upon in the 90s during the time the game takes place.
Appendix carry (AIWB) is a vastly superior choice. Much faster draw, relatively safe with a good holster, and conceals extremely well. The concerned about shooting yourself in a sensitive area is mitigated with good quality holsters and not putting your finger on the trigger inappropriately. Appendix carry in various forms has been used for as long as there's been a handguns of some sort. It's always had an appeal to experienced gunfighters (with a round in the chamber) on what ever side of the law they may be on. And extremely popular with the less experienced criminal crowd (typically carried without a round in the chamber) both now and historically. But it's mainstream popularity and holster product support is a relatively new phenomenon, maybe about 15 years.
Both SOB and AIWB can be accomplished with and without holsters. Obviously a proper holster is the safest option. But note that all the vanilla handguns are relatively safe to carry. The revolvers are all DA with one DA/SA so they have long and heavy trigger pulls. The semi autos all have multiple internal and external safeties. The M9 and the desert eagle inspired vanilla handguns have slide mounted safeties. The 1911 has the frame mounted safety lever and the rear grip safety and is specifically designed to be carried ♥♥♥♥♥♥ and locked in condition 1. And the 90s had similar standards of trigger discipline to what we enjoy now.
Which is all a long way of saying great mod!
Good addition and one that fits the setting and time period of the game well. I don't think we need an ND or AD mechanic tied to this. It's not particularly authentic. But a risk of injury from falling on one's back wouldn't be a bad addition in my opinion. Appendix carry would be great to see as an alternate mod. And new in-game holster items for SOB and AIWB would be pretty cool. If nothing else it would help clean up the side of the character model by not having your belt tools and your sidearms clipping into each other.
Of course you can't wear the vanilla belt with this, so players would be missing out.
Suggestion for a work around- maybe a standalone holster item that allows for an extra item to be placed.
CCW Holster might work as a name, I dunno. Other than that, I love it.
