Bugs
I received both the factionwide debuff and region specifc debuff on turn 2 while taking the last region of my home province (my third settlement).

If it helps, I had a similar issue with the Slow Start version of this mod, but one turn earlier.

Slow Start: Region debuff turn 1 (correct) and Faction debuff turn 1 (bug)
Regular Start: Region debuff turn 2 (bug) and Faction debuff turn 2 (bug)

Also, my 4th settlement, which is adjacent to my fully controlled home province, triggered yet another factionwide debuff. As a result, by turn 3, I've received both Strained Supply Lines and Exhausted supply lines despite never having taken a settlement that wasn't either within my starting settlement or adjacent to it while it was fully under my control.

Based on their control values, I'd say this bug affects only the player, not the AI. I currently have -21 control from the debuffs in my 4th settlement, but Franz has only -10 in his 6th settlement (turn 3). Seems likely that he's got the regional debuff but not the faction debuff.

Not bugs. I said in the description that this version doesnt kick in until youve gotten your 4th settlement or completed your province. This means when you get your last settlement in your first province you will get the debuff, or when you get the 4th settlement you will also get the debuff.

AI franz starts with 4 settlements remember, so he only got two more, and had an adjacent province to reduce debuffs at the same time. When either of these two conditions are met, you will start recieving full debuffs. I.e. you capture your full first province, you get exhausted supply lines for finishing it. If you capture another settlement, you get the strained supply lines (because you now have a single province adjacent), meaning if you capture your last settlement in province then another settlement quickly, you will get significant debuffs.

This is all intended, its called slower expansion for a reason, stop expanding so quickly and you wont get penalised as heavily :p.
