Can't decommission Enterprise?
So the NX-01 is reaching the end of her vaunted career. However, for a while she was on a three-year mission which (randomly selected, I can only assume) took place in the Kaminar system, home of the Kelpiens. During the mission, various shenigans occurred and the Kelpiens broke away from the UFP, leaving the Enterprise as MIA. She reappeared in an adjacent system some time later, but still locked. It's now nearly ten years later, and she's still on this mission and unable to be selected or decommissioned. Is there a way I can fix this bug?

Goto the Federation starbase 1. There is an option to pull all ships back from missions. That should fix this.

I artificially inflicted enough damage to destroy the ship. No leader was aboard, so harm done there. I received no notification, so am I clear to commission a new flagship?

I dont think you will be able to commission a new one now. The reason was you artificially did the damage. The issue with the game(vanilla), is if you do artificial damage, it doesnt calculate to trigger other sub routines within the program to reflect the ship is no longer there. So even though you destroyed it, the game will still see it active. You might get lucky and it will work, but I dont believe so.

So I reloaded the save and tried using the council like you suggested, and it worked--the NX-01 is back on active duty! So when I reach the right tech level, I'll be able to commission the 1701?

I reloaded an earlier save and used the council to recall Enterprise, and she's now back! So when I reach the right tech level (I'm about 2230 or so) I'll be able to commission the 1701?

Correct, it should work properly now. Just remember, there will be 2 techs for the constitution class. Its the 2nd one that allows the building of the flagship for the constitution class.
