The only solution to these kind of problems is to ensure that voting is not worth their while, for example they must spend 12 hours idling on a program that generates a token for them to use to vote, somewhat like bCrypt, or bitcoin or Teamspeak's identity security system. The problem is that such a thing would take a lot of time away from the user.

Hm, we were having problems with the Steam API recently, building up a backlog of requests, that might be the cause. Also, shift+f5 causes a complete refresh regardless of cache in most browsers.

I wouldn't advise that, since that could be considered against the rules (Don't make trades that aren't trades). If someone wants to donate to other players, then they have many places they can go to do it, just not Outpost.

He's probably banned on an account basis- if he does not carry the TF2OP cookie, he is effectively logged out and can browse the site like any guest.
