Long story short, full re-install, slowly rebuilding my modlist, finally found the source of my issues.
None other than a new addition to my old modlist which I won't really miss anyway (and half-started the purge to begin with.)
Even tried launching as minimally as possible without errors/warnings (Core and Ultratechnology) after another full wipe and install, and it's still doing the single settlement generation issue, as provable with setting the worldgen option "Population" to "Crowded".
Ultimately no skin off my back to let this mod go and move on.
generating a new world with this mod prevents more than one settlement for each faction from spawning.
