I have added the option to be notified each time on map entry.
Just pushed an update with an option to turn off the icons.
For sound notification I'd say to use MainText Alert https://steamhost.cn/steamcommunity_com/sharedfiles/filedetails/?id=3395758908 because maybe you also don't want sound for everything and I don't want to fully replicate that mod, but keeping two different filters could maybe be cumbersome... maybe if more people think it's useful I can think about it.
About the map, I'm fine with checking only once. Less risks of bugs (-> less work to support the mod in the future) and script lag maybe. But as @Keira Newmoon suggested my opinion on this is just one among others.
Paying attention at the right time should be rewarded. Ideally the check should depend on the Perception stat of the player to smoothly integrate into the gameplay, higher level targets requiring a higher stat to spot.
