I have added the option to be notified each time on map entry.
Just pushed an update with an option to turn off the icons.
For sound notification I'd say to use MainText Alert https://steamhost.cn/steamcommunity_com/sharedfiles/filedetails/?id=3395758908 because maybe you also don't want sound for everything and I don't want to fully replicate that mod, but keeping two different filters could maybe be cumbersome... maybe if more people think it's useful I can think about it.
About the map, I'm fine with checking only once. Less risks of bugs (-> less work to support the mod in the future) and script lag maybe. But as @Keira Newmoon suggested my opinion on this is just one among others.
Paying attention at the right time should be rewarded. Ideally the check should depend on the Perception stat of the player to smoothly integrate into the gameplay, higher level targets requiring a higher stat to spot.

If people think it'd be better to check every item/character in the map when entering every time instead of only once, I can make that change.
With the new update I tried to prevent notification on world map since it doesn't make sense anyway.

Unfortunately I thought it was working fine due to another version of the mod that I had.
Statue generation (and some other items like potions and furniture) aren't that linear, it uses the thing as a base and makes something else. However, I made the mod check the origin thing as well so now statue_god works fine (although the name on the notification is not exactly the item name, I didn't look into how the name is built yet but it's clear what statue it is).
The other options (altars, shrines, etc.) seem to work as long as there is only one of them on the map. For example,
- 2 altars
- 1 shrine and 1 wheel
in a nefia lead to no notification.
I will try again to play with a different configuration, to force the message to appear in the log, in case I just missed the on-screen notification.

BigDaddy, Statue_Power, and Altar show notifications, but only Statue_God does not.
Is there any other setting I should change?
