-ancient ruins are no longer placed if the No Ancient Ruins option is checked

-improved per-player ancient ruin placement. ruins are no longer placed too close or too far away from starting positions. also, ruins won't appear on resource tiles.
-improved calculation of the number of resources to put in a lake. less possible that a small lake will receive no resources or a large lake all resources.
-fixed bug where rivers could start into snow or desert tiles

-improved river start direction calculation. they now prefer to flow near starting positions more.
-improved player start terrain generation by using normalized multiple masked fractals and terrain percentiles with randomized variability. ;) I.e. terrain around player starts looks more natural while still being distributed is similar quantities
-improved calculation of best start for epic starts. now prefers more balanced starts.
-changed default city-state size to 2 (from 3)
-increased chances of having a river. each lake can possibly spawn 3 rivers now (5% chance per lake tile).
-slight increase to the amount of hills within start areas
-fixed bug where starting units could be on impassable terrain
-fixed bug where natural wonders and resources could be placed in player start tiles
-fixed bug with rivers not going their full length
-fixed map boundary bugs
