if your character is male, the decision should be in the menu when you right click on the portrait of the mystic female character. the option should be there for any mystic female character, though it remains grey and locked until you meet the requirements. after you have received the book, this option will disappear for the rest of your character's life.

if it's not showing in that menu at all then something has gone wrong.

as a last resort you can get the book through the console with the command "add_artifact midwifes_book"

Troubled getting a book, i already have mystic trait female character with 100 opinion and have favor. And no ask about contraception decision

can you be more specific? i just tried it in the latest patch and it seems to work for me.
are you having trouble getting the book or is the book not reducing fertility?

If you have CK3 you can check my attempt @240 Dollars Worth of Pudding its abit more lame then your mod but I try to improve it. Also I finnaly mentioned you. Thanks you for the inspiration.

I just made my own mod which is influenced by your mod. It using the same idea a negative fertility multiplier but its a decision. You mod was alot better but since CK3 has no inventory I cant remake it. And I forgot to mention your mod sry.

i would like to make my mods for ck3 but it will probably be a little while before i get the game, sorry. i support anyone stealing anything they want from my mods if they want to try it themselves

i made it a book specifically so you can toggle it. go into your inventory and unequip the book and your fertility will go back to normal.

the only problem is that your fertility doesn't change instantly. if the game is already paused on an event that would normally result in pregnancy, unequipping the book before clicking the popup will not work. you would have had to remove the book before the event happened.

also any translations would be very welcome, thank you

It works great but I somehow wish I could toggle it on and off (have a baby with my spouse for the sake of inheritance but not with my lover), I dont know, if thats possible.
AND I would like to translate this mod into german, are you interested?

can I come whit a suggestion and that is to lower the male difficulty penalty the better equality there is in the kingdom? at full equality the cost is the same for male/female maybe even have a discount for high legacy+tolerance. representing the book been more accepted in general.

it's just harder to get for males. works for any character the player is controlling, can't be used by ai.
