Q: How do you receive the kill icon for the medkit or anything that you couldn't ordinarily kill someone with?
A: Step on a cockroach from Half-Life: Source, use a kicking addon, or use an overpowered weapon.

also what is the name of the map in the 5th images?
Headcrab: A Headcrab over a head.
Zombie: A zombie hand swiping downwards.
Antlion Soldier: An antlion leg stabbing through a body.
Antlion Worker: An acid warning symbol or a body being dissolved by acid.
Half-Life HECU Marine Guns: MP5 icon and SPAS-12 icon.
Half-Life HECU Marine Kick: A boot.
Ragdoll Slaughter
Both vortigaunts from Half-Life and Half-Life 2 has kill icons. You have to be killed by them in order for it to show up.
As for fall death I don't know how to do that. The icons themselves I can make, but to get them to show up in game, I have no clue.
I can code in kill icons for entities and weapons and stuff, but situational deaths like head-shots or falling to one's doom? I don't know, I'm sorry.
I would recommend using the kill icons made by Neptune who apparently knows how to do situational kill icons.

If I knew how do to that I would. However, gold kill icons will soon be coming to Bills Custom Kill Icons Workshop Edition as a part of a Team Fortress 2 update! :D
Golden frying pans, saxxy awards, and...that's it. Anything that's over 90% gold will receive a golden kill icon!
Unfortunately those are the only two things that will be getting a gold kill icon in the update, however future updates will occasionally give other gold weapons gold kill icons.
