The renaming to Pingyu and the transition to Qiao Commandery likely occurred posthumously, possibly during later reorganization efforts by the Wei or Jin dynasties, but it's difficult to say when this happened, if it ever even did happen during the administration of Wei at all. Likely not until Jin.
So, I think you are just a fan of the KoeiTecmo games, and wanted to rename it Qiao commandery, and the city renamed to Pingyu just because you like it that way, not because you want historical accuracy.

Glad you like it. Actually I did just as your suggestion said in the first place. You can click the first two links to see the Chinese versions of this mod.
However when I made the English version, I found there were too many characters in one commandery, like GUANGYANG commandery with JI and YUYANG countries. If names of commandery and countries all used the same font and direction, it looked not so good.
If it's only slightly bigger and/or bolder with the same font you used for counties I feel like it would give overall map more readability / uniformity and will be more pleasant to look at.
