@jhx007, Settings shouldn't reset after restarting the game. Maybe you have an outdated version of "Mod Config Menu" installed? The current version is called "Mod Config Menu Pure" (link is in the mod description).
Or is there some reason to use "Glowing Hourglass" immediately after "Forget Me Now"?
P.S. Also, just a reminder that no mods now automatically work on rep+ after installing from the workshop. You need to manually copy them to the directory where rep+ mods are stored. There are already a lot of instructions on this topic.
Unfortunately, it's been over two years since I've played the game.
If anyone can give me more information so I can reproduce this bug myself, I'll try to fix it.
Maybe my mod conflicts with some other popular mod?
An option to have them on false whenever you have the Book Of Virtue would be good...

If this isn't the case, try to delete config files of the mod in this directory: "...\Steam\SteamApps\common\The Binding of Isaac Rebirth\data\forget_me_in_3s\". (save1.dat, save2.dat, etc.)
Certainly only a subset of people experience crashes. We need to figure out what is the difference between this part of people and the other.
