The big red grin just looks a bit too goofy to me personally
If you wish for his AoE debuffs to do damage, look at his CoM trinket to add a Blight effect on his Ranged attacks!
Im under the assumption you are supposed to use Malicious so your AoE debuffs do a small amount of flat damage but using Putrified Flesh is doing 0 dmg while still debuffing enemies

the accuracy debuff is not fair
it should be much lower but also have a unique property of reverse scaling with torchlight to help counteract enemies' big accuracy buffs
60% of the acc debuff at full torch then gradual improvement to ~130% at pitch black would be a good idea

But still, it's really satisfied to using him (especially his sound effect).
Something that would help is giving Malform a 140% base stun chance like other stuns. Setting up to land a stun (through debuffing) just doesn't work right in this game, since you want to stun on the first turn, not subsequent turns. Give Malform a normal stun chance by default, and then let him buff it if desired through Excruciating Pain if he really wants to land a big stun on a stun-resistant but not priority target (e.g. Supplicants).
Additionally, I think ALL of his debuffs should mildly boost Malform's damage along with another effect. You could then change Eroded Mind to do something else to Malform (status effect?).
