Seems this needs to be done before world gen.
In the classic version, playing a Human fort was as easy as adding SITE_CONTROLLABLE to the entities text file under the human entry, so I'm not sure if the removal of the vanilla nobility titles was a bug or an oversight for the Steam release. You could probably just remove vanilla_entities from the load order completely to prevent the issue. Alternatively, just make sure you're playing as one of the human civs that has a "Padeshah" listed as their ruler and the nobility titles seem to function as intended.
It looks like the game is treating the Humans from this mod and the Humans from vanilla as two separate types of civs when you include both files, and is generating both types in the same world. Since the vanilla entities file doesn't include the regular human nobility positions for whatever reason (they're literally just missing), it would make sense that you can't assign nobility when playing as a one of those civs.

Yeah you need to do that when you add pretty much every mod except for a few types. You add mods at world generation (for some reason) with a mods button down there as you generate the world.
