Could you share your seed, what other mods you're running, game version (Rep or Rep+), and - if you have it enabled - what your Mod Config Menu settings are? No worries if you don't want to, but without that information I won't be able to verify the issue or resolve it.
It's a simple fix though, I'll get an update out soon. Thank you for finding these issues!
I haven't finished this one yet, so I can tell the seed and other stuff if needed
So if true I think this should only happen on rare occasions, but I'm going to see if the game is OK with me just checking every square in the room for void portals.
But if it is, I don't think it would be worth all the work to implement, given you can already enable or disable the Gauntlet easily from the Mod Config Menu now. G. Normal and G. Hard wouldn't even cover all the difficulty options this mod has.
Hopefully you're doing the efficient trial and error method of disabling half the mods at a time. And if it happens regardless of which half of the mods are disabled, it's probably my mod alone which is causing it.
I did update just this week, so it's perfectly possible it is something I changed which is causing this issue, but that doesn't mean it isn't compatibility related.
I've also noticed another weird behavior. In my last test, the floor order went: Basement I, Burning basement II and Dross I and back to Basement I.
As for my mod list, is there a way to share them? Or do I have to type them out one by one? As i am running 100+ mods, some of which haven't been updated in a while ( but were still working a few months ago)
Does it happen every time, or just some of the time? (E.g. when you resume a run, when you set it not to require knife pieces etc.)
Are you on Repentance or Repentance+?
And when it happens, do you jump into the trapdoor as expected or get the Forget Me Now animation?
Also, could you share the list of mods you had enabled at the time?
It might be due to a recent update from this mod or another, as I haven't played in a few months, and it was fine back then.
I assume that you either have another mod enabled that's conflicting with this, or something very specific happened in your run that I hadn't accounted. If it's possible for me to resolve, I don't currently have enough information.

This has been discussed in the comments previously, but unfortunately there is no way to make it give you the Mega Satan completion mark with this mod enabled, because if the mod doesn't move you to The Void before the mark would trigger, then there's a 50% chance the run will just end there. In general, the purpose of this mod isn't to make it easier to get completion marks, that's just a side effect of it.
Weird that you got the unlocks and Steam achievement without getting the mark though. I didn't know that could even happen.
I just made a run with this mod with Apollyon and i really liked it! One problem is that when you defeat Mega Satan you are automatically are sent to Void, because of this after i beat mega satan i got the steam achievement, i got the ingame unlock (locust of death) but i didn't get the checkmark on appolyon because i think it teleported me before it fully finished the animation.
Now i did have the "Always Void mod" which always spawn a void portal but i don't think that was th problem.
Anyway, i'll probally use the console to get the checkmark since, again, i already got the achievement and unlock, but i'd really like if this was fixed.

After some trial and error testing, I found that the issue is caused specifically by the 'DaBaby!!! Less Goooo' mod on its own (and your seed, items and character weren't a factor either). Do with that information what you will.

So, I've updated it to be compatible. If you have Devil Keys enabled, then you won't be automatically given regular Key Pieces, you'll instead get given both Devil Key Pieces when you beat The Lamb.
The major difference is that, due to me not understanding exactly how the Devil Key familiar works, if you get both devil keys from devils then you can go to Mega Satan as soon as you get to the Dark Room, if you want to, instead of the Lamb fight being mandatory. Not perfect, but it'll do.
(On the other hand, if you reset the floor after opening the MS door, you have to fight Lamb again to get another key, unlike what happens in that scenario usually.)
If it's the former then I'll look into possibilities for making it more compatible, but if it's the latter then there's probably nothing I can really do.
Either way, it might be that a mod that guarantees you'll go through Mega Satan every run and a mod that adds extra restrictions on your ability to get to Mega Satan were never going to mesh.
