Most likely you are playing with a different faction which adds ranks and new family start doesn't specify which faction rank you will have, you have to double check that when choosing pawn on new game
is that intentional?
Im used to them always landing on top of me.
I do not know why this is the case, but if you're experiencing a bug where foes are able to stay alive at 0 second bleedouts and limbs missing, this mod appears to be the root cause of incompatibility with one or more other mods/changes mid save due to updates.
When I try the new family with the entire VE expanded collection and a few race mods, the pawns must come with a Baron title.
The issue is that the "bestower" of this title isn't always the empire, sometimes it's dragonians or a deadman switch faction.
Am I missing some fix or modfix that prevent such issues from happening?

The machanic needs some heavy nerfing then. Makes gaining honor defenitly way too easy once you got 2 nobles.
For now i'l just impse a hard "one noble" rule on myself
I'm not a huge fan myself of this system, because as long as you have a few nobles with good social in your colony they tend to snowball very quickly honor gains. I wish you could tweak it a little.
I'm not aware of any settings or mod to change this behavior but it's definitely NOT a bug.
