That's all on Valve themselves to do. I found a post by asherkin on a CS:S ask explaining how it's a bad idea to force maxplayers above the maximum intended limit, which should also apply to L4D2 using the same engine code to handle players.

Find a mod that increases special infected spawning/limits. I linked two such mods in the description.

It tries to fix the Is<Survivor>Alive context for the extra survivors but other than that, nothing else.

Sorry, mod has not been fixed for multiplayer.

That error usually triggers when survivors are somehow not able to be spawned, in order to avoid an infinite loop of trying to futiley spawn bots. Maybe there were either too many special infected or a variable from a script or mutation was set to stop survivor bots from spawning.

Yep, I got the pistol spam drop with Manacat's Common Library installed. God damn it, I first saw the rngitem script he had in an addon unrelated to weapons but to think this stupid thing was leading me nowhere as I don't use mana's addons.
That seems more likely than weapon scripts right now. The ones I was linked to in the bug reports didn't cause the spam that I keep seeing occur. MANACAT for some reason adds something messing with the pistols to his addons that are not related to anything about the guns.
I don't know why this bug happens, it's never happening to me so I can't replicate and fix it. Other people are getting this randomly that's tied to the survStartWeapons settings that stops once that's erased at the cost of removing the first aid kits the extra survivors spawn with.
My guess is another set of plugins/scripts trying to spawn survivors. Those and this mod likely aren't compatible.
Those are client-side issues that can't be fixed.

After the first chapter, from then on, bots drop two more pistols, while keeping their two equipped.
This mod uses the chat for it's commands, not the console.
