i'm not educated enough to make a guess on this but what i've noticed about this country specifically is that their royal attire changes twice, once when Naser Al-Din Shah takes charge, and then reverts back once his heir takes charge.
Fascist is fine - both an authoritarian presidential republic with a fascist PM or an authoritarian corporate state should work fine

Vic3 1.9 will release this coming Tuesday. As with previous major updates, a 1.9 update for this mod may take several weeks or months because I am a solo developer with a full-time job working 10+ hours Monday through Thursday. Progress during the work week will be rather slow, and I have over two dozen mods to check & verify whether anything needs to be changed, and then make any necessary changes.
The upcoming tweaks to certain things like decisions and scripted buttons will mean that more of my mods will be impacted this time around, so it may take quite some time.

You will get an event asking what to do with the monarch. Choose the option to preserve the monarchy.
Alternatively you can also go to their Domestic tab and start clicking through their IGs and checking their leaders. Whoever the Prime Minister is will have their role (right under their name) say "Prime Minister of <country>" as opposed to "Politician in <country>".
Adding a title to their name in places like the character tooltip is not something I want to do since that would require more GUI or localization changes that I don't want to make in the interest of compatibility with other mods.
I'd recommend checking out the Better Politics Mod for something like that since they have a more bespoke mechanic that'd be better suited for handling what you're asking for.
Crowned Republics can be formed with Corporate State.
Issues related to the Divided Monarchists journal should also be resolved now.

I don't use BPM so I will still maintain this going forwards as long as I don't quit Vic3.
RRK does have special modifiers for legitimacy if you have a Prime Minister/Premier with a different IG than the monarch, though. Putting the PM's IG in government will grant you a similar legitimacy bonus in lieu of the "head of state's IG in government" bonus in that case, although it requires you to make the actual government reform first (i.e. it's not reflected in the previews).
Just FYI I will be busy with family things for the whole of this week, so I will not be able to get a 1.8 version out (or do a 1.8 compatibility check on the existing version) for launch day or for several days following the update. Sorry for any inconveniences this might cause.
