I've always wondered why they rarely ever showed up.

It maybe could cause issues if there's suddenly a large number of injured world pawns (like maybe a large raid fleeing the map) but it should stabilize once all the injures are tended.
If the game's performing these calculations more often for every world pawn, it could, couldn't it?
Or, if it's all done at once for every pawn anyway, if there's any performance drop it would be a spike, and this would just make the spikes more common?
