1. Play around the restrictions. Cards that allow you to have more cards in your turn are fantastic: Hello World (don't sleep on this!!), Compile Driver, Equilibrium, etc.
2. Ideally, you should try to keep at least two orb types at all times to keep your Compile Drivers viable.
3. Taking orb cards is generally a trap UNLESS you have either (Runic) Capacitor/Inserter to get more orb slots.
I do think the challenge might be SLIGHTLY too difficult. I just gave it a shot on A20 and did die to Corrupt Heart, but I wouldn't have even made it that far in the first place without some of the lucky relics I got such as Shuriken + Meat on the Bone. If I update this mod again I'll probably keep the challenge as-is, but move it further down the Defect list to indicate that it's a more difficult challenge.
(also, Necronomics bans Snecko Eye but does NOT ban Enlightenment. I may have to change that. Hmm...)
For exemple, in the Necronomics challenges (Ironclad where you start with Necronomicon but every card you play costs 1 more every time you play it), I don't know if I can find an Enligthtenment (0 cost card that sets the cost of your cards in your hand to 1 for the turn/combat) in a shop or not.
It would be helpful to have this kind of information

I think the only possible concern now is that you could have so many variables in the challenge definition (lots of cards, lots of special rules, etc) that it goes off screen a bit, but even my own most complex challenge, Duet, doesn't do that, so hopefully that isn't an issue. If it is, I'll push out yet another update, maybe by making the starter cards in two columns rather than one or something.

As for The Sadist, I am aware that's a pretty easy challenge. However, I think it's fine if some challenges are less difficult than you'd expect, especially for The Sadist, as it's the first (or introduction) Silent challenge. The Binding of Isaac, which this mod was heavily inspired by, similarly has challenges that aren't necessarily hard but encourage you to adjust your playstyle which I think is neat.
Challenge selection is messed up across different save slots.
And a suggestion about balance:
The Sadist is too easy to be counted as a challenge in my opinion, even on A20. Maybe removing Twisted Funnel could help?
It might be fun to face the City with initial deck, but definitely not so for the Beyond.
