Yes, that is explained in the description. you must start the game with random leader, you will start as Cyrodiil. Then as soon as you enter the game, you can Exit to Main Menu. Now once you load the scenario again, you can choose whomever you like................It has to do with the way Civ V loads mods, and that is the only workaround.
Not until Firaxis makes it possible, though in a recent podcast I listened to, Firaxis said that getting mods into multiplayer was at the top of their list for the "Fall Patch". So maybe in a couple months, when they release the patch, we'll be able to use mods in multiplayer! Fingers crossed :)
I havent tried this, but I think you may be able to go into your Documents/My Games/Sid Meier's Civilization 5/Mods/The Elder Scrolls Civilization V Pack (BNW) (v1)/XMLs/Texts and find the "CoreGameTextUpdates.xml" Move this file out of the folder (place it on your desktop or something where the game cant find it). That should work, I puposefully placed all the "name changes" into a separate file for this reason. That way, when you do want to play with the name changes, you'll just move it back into the same folder.
(Small chance the mod wont load at all, because of missing files, but unlikely) I'm thinking that it will just play on and forget about the name changes.
If that doesnt work, then I'm afraid you'll just have to disable this mod, while playing other mods.
Edit: yes all that file contains are the name changes for city-states, religions/beliefs, resources, world congress, eras/time, Barbarians2Bandits etc. (so moving it in and out of that folder, shouldn't be a problem at all)
