Also, a few questions to help me diagnose:
-Does the issue happen when a summon/totem already exists, or at any time?
-Does the issue happen while in combat, outside of combat, or both?
-Are there any graphical issues associated with the problem?

Although i did use a lot of mod, the totem works perfectly fine. However, if i recalled correctly, the problem happened when I was fighting the houndmaster.
In the beginning everything was fine, i summoned water and wind and fire totem.
While in the middle of the fight, when I was trying to summon a second wind totem, the totem immediately dies.
After that whenever I tried to summon totem in or out of combat, it died immediately as if it was destroyed. No graphical issue.
I am now in Act 2 now but it still doesn't work.
I though it might have something to do with "mimicry" mod because it gives totem a buff called rage. However if it is conflict of mods then I suppose it shouldn't work at the beginning (maybe? lol)
Mate, calm down, you posted this exact thing to the other mod as well.
Because of the nature of the glitch, however, those who are currently affected have to be "reset," so to speak (tracked via Flags in the code). I've added in an easy reset, though, as one simply needs to enter into any combat and it should become resolved. Thanks again all.
I just tested Shattering Throw and I can agree it also does nothing other than use up the source point and AP cost. Hopefully Feenex can take a look at both Ruthless Assault and Shattering Throw soon, I've been enjoying the barbarian skills so far.
I don't know exactly what causes it but I've notice it will more often than not, cast after I've used Shadow Bolt and follow up with another spell (not cast at the time) and then let the initial Encore cast. The Second cast seems to be delayed though.
