I didn't realize there was a bug forum when I posted this as a comment. Copying here for tracking:

I saved the game after the second barfight (when the strippers come after you) but I died. When I told it to load my last save it put me back in the bar with three hellhounds attacking me. I managed to kill the hellhounds but now I'm stuck in a permanent fight with nothing and no way to leave. After loading that a couple of times the hellhounds went away and I was in a perpetual fight with no enemies, and all the NPCs loaded in.
