I was going to post this a couple of days ago but kind of forgot, I have another thing to add on this as it happened again in a new game. Almost all of these in both games seems to have involved born in the purple trait as either the person who inherited or their mother(or in one case both) had the trait born in the purple. While I can see no reason why it should effect any succession beyond the Empire's, especially the mother having that trait, this seems to be the case.
For the mothers having the trait: I've seen several children whose mothers had the trait born in the purple get the primary title of their father instead of their elder half brothers whose mothers didn't carry the title. It didn't happen with all the mothers with born in the purple trait but in all the cases of it happening that I've spotted, the mother had born in the purple trait. I've checked title history to see whether they were installed by faction demand etc and no, they inherited straight from their father, passing over their older brother.
For the children having the trait: I can't remember the examples for this from the game that made me post this possible bug, (although there were two cases in that too) but in my last game I have another example of this happen:
My wife(who was also born in the purple) became an empress, we previously had a son who was born before her becoming an empress and we had another son who was born after her becoming empress. Once born, our son with born in the purple trait became my primary heir over his brother.
Edit: forgot to add, my wife lost the throne some 10 years after his birth and he was still my heir, I've decided to check whether it will indeed be the case because sometimes game doesn't update the list of succession shown to the player (although he shouldn't have become my heir in the first place) but when succession happens it happens correctly and well... son with born in the purple trait got my primary title.
