It's a known bug in the original game toolkit. If you place the black screen text after the loading screen transition it will work as expected in single and multiplayer.
The two screens are:
https://imgur.com/a/7kSooOd - in Zarr-Xuul's temple, when you go down into the hidden catacomb
And at the end screen of the game, making it impossible to export your characters and levels (granted, that's not an issue, there aren't any campaigns that run above or start at level 16).
Sorry for the confusion, but to be clear, this is a vanilla bug too I'm pretty sure, impacts leveling up twice in multiplayer. You can test it pretty easily by launching Palace of Ice (built-in campaign) on a vanilla game in multiplayer and doing the built-in level up option with new characters. Causes it to error out and can't level up (at least on PC, not sure about console). For PoI and other campaigns that start at high level, you can work around by using exported characters already at the level, or by leveling up on your character page in the main menu.
I was going level 15-17 at the time because of the mod allowing higher levels, but the same thing will happen if going from 14-16 with base game levels if you'd skipped some side quests earlier on.
Ah... actually, I wasn't aware of any of that, so that's on me. Given the town was wrecked and full of undead I wasn't assuming any rest spots were still up in the main town area, and given the urgency level of the quest (and that we couldn't go back into the inn), I went straight through to the duke's place, and wasn't able to do a rest until all the way in the guest rooms upstairs across from the duke and family. Honestly not sure how I missed the one in the Stronghold Courtyard, but I definitely didn't see that at all. That's really weird, that wasn't even a large map area and I'm always pressing alt to check for highlights.
OK, I understand the bug is there on vanilla too, I only thought that you 'suffered' this bug because you were still levelling up beyond level 16 thanks to the mod, otherwise if you were capped at 16 you wouldn't have to level up twice. But yes, could've happened anyway to characters being lower than 15th level at that point.
Well, kudos to you for roleplaying the urgency even though the game itself has no 'timers' to be worried of. The resting places are there, but I left them quite 'hidden' to convey the mood of a dramatic invasion. You know, you can force your way through, having to manage your resources without any rest (as you did) or dare some more exploration hoping to find a safe spot but risking more encounters and more waste of resources.
But I definitely placed the one in the Courtyard (in the zone behind the barricades where the guards were fighting demons) as a well deserved rest after the big battles and before entering the stronghold, (and it made sense there, because it was a sort of 'camp').
Again, thank you for your detailed feedback, I have already updated the campaign with tweaks to the lore exits that were interfering with multiplayer.

Thanks, and much obliged. Was a really fun campaign.
Don't forget to click the 'thumbs up', if you wish to do so. Thanks.
Done, and thanks again!
