Yeah your Load Order is ♥♥♥♥♥♥ up. You shouldn't be moving mods around like that with MW5. This isn't Skyrim, the LO should be 99% what their oLO is. Reset everything to it's default position.

You've also got mods that have the wrong original load order spots like my Yet Another Weapon Clan - Addon and Fixes. It's oLO is 1, but in your list it's oLO is 40. I'd actually recommend a clean reinstall of all your mods to make sure they all have their correct oLO (original load order) numbers saved.

Ok, reset mods to default, validated game files, started a new career, still have the same issue. Updated load order in my first post.

Ugh...Just started a new game using ONLY this mod, still having the same issue. Minimap is a big square, overlaps the lance orders, and the lance orders are pushed up over the top of the screen.

I just completely uninstalled all my mods and re-downloded them, since you said the oLO of some of them were not correct. Testing again.
