* Increased amount of temp HP gained from kills on Biohazard from 0.75 to 1.5

* Difficulty scaling variables with decimals were broken when using ((difficulty + 1) / 2) format. Eg. Stinger projectile dmg was 0 on easy (0.5x), 5 on normal (1x), 5 on advanced (1.5x), and 10 on expert (2x).
* Setup health scaling for ferocious/monstrous cards
* Removed some unnecessary difficulty checks
* Removed Gamemode Options as it is redundant now
* Simplified the way scaling is done (?)
* Limit Biohazard & Frigid Outskirts to higher diff.
