1) Can you try disabling mod cache and trying again?
2) Are you able to access the link directly in the web? (It is the PDF export of Niels' cooperative rules)
If it loads for you, @SamuraiJackJr, then it's probably still in your cache. The best thing to do is open the link in your browser, and do a hard refresh (Ctrl-F5) on windows. If it doesn't load there, it definitely won't load in game.
The blue Quest Deck is called the Cross-Faction deck in options and the Destiny deck when mousing over it. The official name is the Blue Quest deck, but you could also call it the Neutral Quest Deck or the Independent Quest Deck. Calling the option Cross-Faction is super confusing. I had no idea that that's what you meant by it until after I tried out what it did. Cross-Faction makes me think alliance have a chance to get horde quests and vice versa. I'd highly recommend renaming it.
Is the DK from JustTobby included? If so, I cannot find it.
The +Green Die, +Blue Die and +Red Die buttons don't seem to actually add a die, but the success effect of a die. Perhaps clearer labeling can help. "Green Hit", "Red Hit", etc..
When playing without outland, the layers over the level 6 area on the player sheets are quite ugly. Would it be possible to simply remove that section when playing up to level 5?

1) Fixed in the update coming out tonight
2) Wow, I'm not sure where I got the more complex name AND I will change it to Blue Quest in the next patch tonight.
3) JustTobby32's DK is NOT currently in; I am refreshing all of the variants through version 1.3 deck-by-deck and upgrading the visuals as well; currently, the Druid, Hunter, Mage, Paladin, Priest and Rogue are up-to-date; Shaman, Warlock and Warrior are old (version 1.1/2); Death Knight is missing and will be added after the Shaman, Warlock and Warrior
4) Great call - I will update that with the next patch this evening
5) That is on my feature roadmap; I actually have to replace the entire board, but did find some original board scans to leverage. I will expedite that feature request after the variant decks are complete
The recent release should address the (4) bugs you mention above.
The other two items are on path to be released next (first variants, then base player board when not using Outlands).
Also, the Shaman update is complete.
