The hiders spawn on the info_player_start entity and the slasher spawns on the info_player_deathmatch entity.
If you want to make objects to hide in (e.g. closets), you place a prop and name it something like hr_hide_REPLACEME, place an info_target in front of it named hr_hide_REPLACEME_outpos and place another info_target inside it named hr_hide_REPLACEME_inpos. Make sure that none of these use the same name. You might also want to place a brush using the tools/toolsblack texture inside the prop if it's not hollow.
And the elevator is completely made in Hammer Editor.
