Hey there, Yvan88!

From your posts, I'm not sure I understood which part of the process is proving difficult. What you said is correct: right-click the control panel, paste the link to the HDRI image in the Description, then press the Background button. After that, make lighting changes that you would like (via the Options button at the top of the screen, in the Lighting section). When you're done, right-click the control panel again, type a name into the Description, then click the Save button.

Right-click the control panel, paste the link to the HDRI sphere image in the Description, and press the Background button (that'll activate the image). The right-click ???? where ????

If you have the image hosted somewhere, yup! Right-click the control panel, paste the link to the HDRI sphere image in the Description, and press the Background button (that'll activate the image). The right-click again, type the name you want in the Description field, and hit the Save button. That's how I made all the built-in entries. : )

I don't suppose there's a way to add new background images?

Understandable, thanks for letting me know!

As far as I'm aware, Taco, no, that's not possible, simply because those variables aren't exposed to script (at least, as far as I can tell from reading the API). If I'm wrong about that, I'll gladly add them to the list of settings that are saved and loaded. But this page seems to suggest that's not possible right now: https://api.tabletopsimulator.com/lighting/

Very handy tool, would it be possible for this to also save the LUT selection and intensity?

I am not a fan of most custom backgrounds, because the sphere is so small and moving around the table breaks the immersion of the background quickly - but that is a game problem and not your fault. Great selection here, so good job making this.
